Atypical Depression

A subtype of depression where mood temporarily improves in response to positive events, often accompanied by oversleeping, overeating, a heavy sensation in the limbs, and heightened sensitivity to rejection.

Atypical Depression

Atypical depression is a specifier for major depressive disorder defined by mood reactivity — the capacity for the mood to brighten in response to actual or potential positive events. Despite this reactivity, the underlying depressive episode persists.

Core Features (DSM-5)

  • Mood reactivity: Mood genuinely lifts when something positive happens
  • Leaden paralysis: A heavy, leaden feeling in the arms or legs
  • Hypersomnia: Sleeping significantly more than usual
  • Hyperphagia: Increased appetite, often with carbohydrate cravings
  • Rejection sensitivity: Long-standing pattern of interpersonal sensitivity to perceived rejection, causing significant impairment
  • Distinguishing from Melancholic Depression

    In melancholic depression, mood does not improve even temporarily in response to good news or pleasurable events. Atypical and melancholic specifiers are mutually exclusive in DSM-5.

    Treatment

    SSRIs and MAOIs have shown efficacy; MAOIs have historically demonstrated particular effectiveness for atypical features.
